Inositol. Strength training. Low GI carbs. Magnesium. Spearmint tea. None of these are bad recommendations. They're just recommendations written for someone else.
In 2025, a study of nearly 12,000 women across five countries confirmed what I'd been seeing in clinic for years: PMOS isn't one condition. It's four completely distinct hormonal subtypes, each with different drivers, different symptoms, and different responses to treatment. What clears one woman's acne and gets her cycles back does absolutely nothing for another — not because she's failing at being unwell, but because she's treating the wrong thing entirely.
Most women with PMOS have spent years following advice that was never built for their version of it. Some of them for a decade. Some longer. And the particularly cruel part is that the advice isn't wrong, exactly — it's just wrong for them. Like being handed someone else's glasses and wondering why everything's still blurry.

The four types
Each type has its own hormonal pattern, its own symptoms, its own bloodwork profile, and its own protocol. Finding yours doesn't just explain why nothing has worked. It explains everything.
Type 01
The metabolic type. Insulin resistance as the dominant driver — weight that sits around the middle and won't shift no matter what you try, energy that falls off a cliff after lunch, sugar cravings that feel less like a preference and more like a hostage negotiation. The most common type, and the most responsive to the right lifestyle changes. The emphasis being firmly on "right" — because the wrong changes, and there are plenty of well-meaning wrong changes, make this type worse.
Type 02
Lean. Bloodwork looks basically normal. Cycles went missing and nobody could quite explain why. This is the type most likely to be told to just relax more (while suppressing the urge to flip a table) and the type most likely to wait years for a diagnosis, because the markers that flag a problem on a standard blood panel simply aren't there. They're just quietly dismantling ovulation in the background, completely unbothered by anyone's attempts to find them.
Type 03
High AMH. Lots of follicles. Ovulation tests that are positive for days, or refuse to give a clear peak at all. The issue isn't eggs — there are plenty of those, thank you very much. The issue is the signal to release one, which isn't getting through. Extended fertility window, later menopause, but a cycle that won't cooperate until someone understands what's actually going on hormonally.
Type 04
High testosterone and DHEA-S. Acne that definitely didn't end with your teens. Hair where you don't want it, thinning where you do. Cycles that disappear for months at a time without explanation. The symptoms are loud and visible and exhausting to manage — and the standard response has almost always been "go on the pill," said by someone who had already mentally moved on to their next patient before you'd finished sitting down, with no explanation of what's driving it or what you're supposed to do about it long term.
